Research Update: Hoof Wall Separation Syndrome in Connemara Ponies

Few hoof problems have been as compelling as the relatively-recent one known around the hoof world as "Connemara hoof wall separation syndrome". Owners of the affected horses would prefer that it be called "hoof wall separation syndrome".

Wall separations are commonly seen in the heel area of hooves, but this is different.

At first, it was easy to dismiss the communications as white line disease or just poor hoof maintenance. But they kept coming back. Soon they had amassed evidence of Connemara ponies with this problem from places as far afield as New Zealand, Finland, and Canada.

Robert Eustace of the Laminitis Clinic in England described the problems as "coconut matting hooves". I thought that was an apt description; in some cases, the inner, exposed hoof wall looked like steel wool fibers. Many cases of this type of dorsal hoof wall separation deteriorate to a diagnosis of laminitis.

According to the owners' group, hoof samples of affected Connemara ponies referred to Eustace for treatment were analyzed by the University of Edinburgh, where they found a malfunction of lipid metabolism in the extracellular matrix of the hoof wall between the tubular structures of the hoof wall.

In simple terms: there seems to be a lack of the 'waterproof glue' or 'cement' that holds the hoof wall tubules together, but the findings have not been documented. Nothing about the condition has been documented, other than by horse owners comparing notes.

Soon, the inevitable Facebook discussion group popped up. It became part of a web of information and communication that included a blog, press releases, and many promises that they would get to the root of the problem.

They may not have reached the root but they are cracking the case. If you have a pet problem in the hoof world and you are sure that there has to be an answer, let this group of dedicated horse owners give you a model to follow.

This hasn't been easy. On  November 11, 2011, I interviewed a regional president and another officer of the American Connemara Pony Society who assured me that they had never heard of the problem and it was surely just a misunderstanding. A veterinarian concurred.

In their defense, this problem is widespread geographically but still relatively uncommon. Since then, owners tell me, the Society has become aware of the problem. It is also possible that owners whose young stock exhibit this problem are not likely to bring them to shows or to comment much about the problem, in the interest of the value of their other horses.

The Connemara owners scattered around the world with lame young horses on their hands eventually found each other, thanks to the Internet. They also found someone who would listen to them. The unraveling of the horse genome has meant that equine disorders that seem to be breed-specific can be examined on the genetic level, so researchers at the Bannasch Laboratory at the University of California at Davis were able to tackle the problem from that angle. When a case cropped up near the university and they saw the problem firsthand, they were even more interested.

Is this problem really limited to a single breed? I remember old farrier texts that complained about mice nibbling on the hooves of certain horses at night in the stable--that was a convenient explanation to owners for a problem that looked like the photos you see in the slide show.

A confounding aspect of the problem is that it occurs in radically different environments, without respect to wet or dry, hot or cold, and appears early in the pony's life.

One Connemara owner remarked, "I am seriously starting to think that this issue is probably quite widespread in the general horse population but appears so randomly that it is acknowledged just as 'bad feet with no known cause'. It is only when one has a small inbred population with the over use of certain bloodlines in an already contracted gene pool that the problem becomes more common and obvious.

"Once there is a commercial gene test available, it will be interesting to see whether there is the same or similar problem in other breeds."

Because the cases are farflung, documentation of treatment is sparse. In most cases, some combination or variation of supportive hoof orthotics, foot casts and glue-on shoes along the lines of laminitis treatment are used. Various topical treatments and nutritional supplements have been used. These efforts are under the heading of managing the condition; euthanasia is common.

Researchers at the Bannasch Laboratory at UC Davis report:

Hoof Wall Separation Syndrome (HWSS) is an inherited condition seen in Connemara ponies and typified by the dorsal hoof wall splitting away from underlying structures.

This hoof defect develops in young foals between one to six months of age. The condition results in afflicted ponies having to support weight on the sole of the hoof instead of the dorsal hoof wall. 

Affected animals can become severely painful despite careful management; their quality of life can diminish and euthanasia may be necessary. Even if the condition is initially controllable, ponies may still develop laminitis over time.

HWSS is particularly troubling for the Connemara community because the parents of affected ponies are themselves completely unaffected. 

Although affected individuals do share common bloodlines, it is problematic to predict whether a particular breeding will produce a foal with unhealthy hooves. Investigation into the underlying genetic cause of HWSS has the potential to inform these breeding decisions, and could also provide insight into the disease pathophysiology.

To investigate the underlying genetic cause of HWSS, a genome-wide association study was performed and a strong association between disease status and polymorphisms in a two-megabase (Mb) region of the genome was observed. Sequencing of candidate genes within this region is currently underway, and any functionally relevant genetic differences identified will be validated using a larger sample set.

Hallmarq Standing MRI and Chronic Equine Foot Lameness: What’s Going On Inside the Foot?

Sport horse lameness requires a diagnosis to pinpoint the injury site and a prognosis to predict when and if the horse might return to training. The veterinarian chooses from a set of alternative plans to gain recovery based on restricting the horse to stall rest, turning him out, or following a prescribed limited exercise program. The program is determined by the site of the injury and the clinician's knowledge of and experience in successfully treating that injury site. If the injury isn't properly or fully diagnosed, facets of the recovery plan--including medications and adjunctive hoofcare--might not be effective, time may be lost and the horse's chances at returning to his former level of performance are jeopardized.

The hoof capsule is the horse’s best friend: it’s a protective covering, a shield against rocks and bumps, and a tractable aid to locomotion. If the horse had to gallop on a soft digit, could he gallop at all?

The tough shell of his hoof wall is the horse’s best friend--until something goes wrong inside the foot. Then it takes sophisticated imaging equipment to see what’s going on inside that wall, and beneath that sole and frog. When we want to know what’s going on in there, it’s time for technology to take over.

A recent study at the Marion duPont Scott Equine Medical Center in Leesburg, Virginia delved into some details of cases of chronic, unresolved foot lameness. Might these cases have something in common, besides that the horses were all lame in one front foot? The researchers wondered, and compared the horses’ records.

While many studies have logged in data about test-case horses with specific acute injuries revealed only by MRI--deep digital flexor tendon lesions in sport horses are prime examples--this study looked at a larger population of horses that had been lame for some time (from three months to five years) and which had not received the benefit of an MRI before they were referred to the hospital for further testing.

Each of the horses had responded favorably to a palmar digital nerve block in the lame foot, so the clinician was confident that the horse’s pain source had been isolated. MRI was performed if clinical and radiological findings did not provide a definitive diagnosis and if the owners elected further examination.

Another thing the cases had in common is that, after referral, each of the horses had been scanned using the same MRI technology: the Hallmarq Veterinary Imaging “standing MRI” system installed at the hospital in Leesburg in 2007.

Horses with chronic forefoot lameness were selected for the study. A Hallmarq standing MRI scan was employed to look for damage to soft tissue structures in the coffin joint and navicular zone.
The veterinarians selected 79 horses that fulfilled the selection criteria for the study and began to compare their records. The biggest thing that the horses had in common was that the MRI scans revealed that almost all of them--74 out of 79--were actually showing more than one structure in the foot with an alteration.

In most (52) of the 74 horses with more than one change, the multiple abnormalities were not of equal severity.

What types of injuries did the MRI scans reveal on these chronically lame horses?

Hallmarq mutli-image with logo• 78 percent of the horses had a navicular bone lesion in the lame foot, along with another injury;

• 58 percent of the horses showed evidence of navicular bursitis (inflammation to the bursa, or fluid-lined sac between the navicular bone and the deep digital flexor tendon at the back of the coffin joint);

• 54 percent of the horses showed evidence of damage to the deep digital flexor tendon;

• 53 percent of horses demonstrated some degree of effusion (excess fluid) of the coffin joint; and

• 39 percent of the horses were suffering from collateral ligament desmopathy of the coffin joint.

Who were these horses? Breeds included Thoroughbreds (24), Warmbloods (36), Quarter Horses (9) and others (10). Occupations comprised jumping (37), eventing (12), dressage (17) and pleasure riding (13). The mean age was nine years old, but ranged from four to 24.

What’s the take-home message from this research?

If you’ve been reading the Hoof Blog for a while, you already know a good bit about Hallmarq’s standing MRI technology, which is now available in dozens of veterinary clinics around the world. MRI was the next level of diagnostic imaging for horses whose injuries were not revealed by other systems or, in the case of navicular changes visible on radiographs, would benefit from a more detailed evaluation.

Not so long ago, we would have labeled these horses as simply having a chronic foot pain condition or, more likely “navicular disease”. Farriers would try different shoeing techniques or trims, hoping to hit on a magic combination of support and comfort. Pain medication, coffin joint injection and perhaps even neurectomy would be on the menu. Some horses recovered sooner, some horses recovered later, some horses stayed lame.

Having access to MRI is much like calling in Sherlock Holmes when the police have failed to solve the crime. MRI results can often be surprising, as we see here, when the scan is expected to yield a given result and does, but also reveals more information than anticipated.

The deep digital flexor tendon (arrows) is a common site of injury in sport horses. While some injuries or abnormalities of the navicular bone can be seen on radiographs, lesions to the tendon require imaging that reveals the soft tissues of the inner foot.

The identification of multiple injury sites in the foot is critical if the lameness is to respond and recover. In 2003, papers by Dyson and Murray in Great Britain and by Schneider and Sampson in the United States used MRI to document that equine foot pain often can be traced to multiple structures. This important new consciousness has been a critical factor in the growing appreciation of using MRI as a diagnostic tool, and the Virginia study certainly confirmed those findings in a large number of horses.

A benefit of the standing MRI system for evaluating horse foot lameness is that the horse is not required to undergo anesthesia. The horse stands on all four feet throughout the procedure, which may be performed as an “out-patient” type of appointment at a vet clinic that is equipped with the Hallmarq system.

This research was part of a series of MRI-related studies on foot lameness conducted by Santiago Daniel Gutierrez-Nibeyro, DVM, MS, DACVS as a component of his Masters in Science degree. The entire set of research papers is a tremendous asset to anyone wishing to understand how MRI imaging fits into the bigger picture of equine foot lameness diagnosis and treatment.

Review: This is much more than a book about barefoot vs shod. In fact, it’s not about that at all. This is the new way of thinking about care for the foot--care takes on a double meaning. Instead of just compensating for the horse’s problems, look at how to help the horse grow a better foot. Think ‘rehabilitation’ instead of ‘compensation’, when you can. It might make people turn their heads, especially  for work on young horses--the idea is to anticipate problems and avoid them instead of fix them.

We live in an age where collaboration is not just an interesting undertaking--it's almost a requirement. Pete Ramey has wrapped his reference book in a cloak of collaborative experts to create a synergistic look at how hooves can be nurtured back to health--or rehabilitated to a state of health that the horse may never have known in its life.

Forget what you know about barefoot trimming and be prepared for the launching pad to the next era of collaborative hoofcare technology. Ramey admits that he has softened his stance on shoeing, but believes we can do better than continuous shoeing with steel or neglectful long intervals between re-shoeings. This book is a snapshot of where we are now--with bare hooves, booted hooves and metal/aluminum/plastic shoes creating the spectrum of choices. It may not be what we choose, he concludes, but how we use the materials and methods.

I like the way that this book opens the door to the future--and leaves it open. Innovation is the path through the door and this book should encourage everyone to decry neglect, ignorance and lack of skill as hooves worst enemies. Some horses may be well through that door, thanks to having new materials and methods and smart, skilled people on their side. I hope in our quest for innovation we don't create a divided society of "hooves" and "hooves not". The best solutions will be the ones that are affordable and usable in many hands.

Ramey also includes veterinary support, imaging, nutrition, pasture management and locomotion in his spectrum; without working them into the hoof equation, the rehabilitation is not going to be realized. Collaboration is not an option, it's a requirement. The hoof needs to feel the influence of much more than a rasp and a knife to find its way along recovery road.

While collecting photos of farriers during World War I, I passed over this photo several times without realizing how interesting it was. It took a magnifying glass to appreciate this one.

The sign reads, "'Kindness to animals, 500 horses lamed weekly by nails dropped on roads and horse lines by cookers carrying firewood with nails left in. Please remove nails."

As if the war horses didn't have enough to worry about with staying alive in combat and battling environmental conditions like mud and heat and lice and mange, and diseases like glanders, they had to walk across scorched earth littered with shrapnel, and sometimes even the "friendly fire" of nails dropped from wagons hauling salvaged timber to burn in the cookers.

What's a cooker, you might ask? A cooker was a horse-drawn kitchen--a sort of wood-fired stove on wheels. Here you see some Canadian troops from Newfoundland who were happy to belly up to a cooker as if it was a modern-day urban food truck.

Puncture wounds from shrapnel and nails are still a problem for horses and donkeys in war zones. They are also a problem for horses after natural disasters like tornadoes, earthquakes, tidal waves and hurricanes.

Hoofcare University: Biomechanics of Racehorses' Lower Limbs and Track Surface Interface Video Lectures with the University of Guelph's Dr Jeff Thomason

Go get a cup of coffee or a sandwich or start popping the corn. Hoofcare + Lameness is happy to share a new video series with you from the University of Guelph's Jeff Thomason BA, MSc, PhD, professor of biomedical sciences at Guelph's Ontario Veterinary College.

Dr Thomason is widely known for his research, which currently looks both at the biomechanics of the horse's foot and distal limb as well as the mechanical properties of the track itself, and how they influence the function of the foot.

Recently, Dr. Thomas was part of a group of authors that created an extensive "white paper" reference document for the Grayson-Jockey Club Foundation's Welfare and Safety of the Racehorse Summit on this type of dual research, which will ultimately benefit the safe travel of horses over optimal racing surfaces.

The Real Mustang Trim: AAEP Task Force Recommends Hoof Trimming for Captive Wild Horses in BLM Facilities

Self-help expert Dr. Wayne Dyer has a saying that seems so critical to me that it has made the place of honor in my life: it's stuck with a magnet on my refrigerator.

It's been there a while, and it's faded and curled but I know it by heart: "When you change the way you look at things, the things you're looking at change."

It's there to remind me about analyzing difficult situations, but a press release I received today made me chuckle. What about when a difficult situation is analyzed by people with completely fresh eyes? When you read an article about your hometown written by an out-of-state journalist, do you think, "Wow, is that really where I live?"

That's the power of a fresh set of eyes on something you know so well; it sounds completely different. Sometimes better, sometimes worse, but you stop and think about the way your little town looks to someone "from away", as they say in Maine.

So what would happen if one of the hot-button situations in the horse world was visited by a task force of veterinarians, most of whom live and work worlds away, either on another coast or in academic clinic settings? Would someone who may have little or no exposure to wild horses see them differently from someone who lives and works with them everyday? Is one vet's status quo another vet's no-no?

When a veterinarian like Dr. Ann Dwyer from upstate New York took a look behind and into the pens at the Bureau of Land Management's wild horse warehouse system in the United States western states, did she see things differently than the BLM's own staff veterinarians and stockmen?

Maybe she did, maybe she didn't, but today the American Association of Equine Practitioners' Bureau of Land Management Task Force released its evaluation--and some critical recommendations--designed to improve the care and handling of the nation's wild horses during gathers and captivity.

The AAEP report comes at the request of the Bureau of Land Management, which asked the AAEP in June 2010 to evaluate the handling, health care, and welfare of the horses and burros at BLM wild horse and burro gathers and holding facilities.

The AAEP agreed to lend its expertise and a task force was formed under John S. Mitchell DVM, AAEP president elect and Task Force chair, who is as removed from the west as Dwyer. Mitchell is a Standardbred racehorse specialist in Pompano Beach, Florida, and the task force he headed was a fascinating cross-section of the United States equine veterinary community. The report notes that one task force member "had visited a BLM facility previously" and another "is a member of a group practice that does contract work for a BLM short‐term facility."

From the academic sector came Texas A&M's William Moyer (who is also currently AAEP president) and equine lameness specialist Professor Kent Carter. From the University of Georgia, Susan White, Professor of Large Animal Internal Medicine. From industry, the task force chose Rocky Bigbie, Senior Veterinarian at Pfizer Animal Health. In addition to private vets Dwyer and Mitchell, the task force included some veterinarians from western states: Jacy Cook, private vet in Bozeman, Montana; Roger Rees, owner to a large animal clinic in Utah; Stuart Shoemaker, a surgeon who left Louisiana State University to start a sports medicine practice in Idaho and Beau David Whitaker, an equine lameness specialist vet in Texas with a penchant for humor writing.

Surely all or most of these veterinarians could be considered excellent judges of western horses. In their practices, they probably see well-bred and valuable cutting and reining horses, or tend to the needs of wealthy ranchers. The horses they would see in their work for the task force would be the other end of the spectrum, America's ultimate "Unwanted Horses".

If you need any proof of that: The United States Government can't even give these horses away lately. Adoption statistics have plummeted in spite of higher visible of mustangs in the show ring. Yet these horses have powerful friends in high places, like a Congress charged with upholding a federal law, Public Law 92-195, the Wild Free-Roaming Horses and Burros Act of 1971, which was designed to protect the horses.

They also have a few million fans who are happy to hold the BLM's feet to the fire, so to speak, over the care that the horses receive in captivity, and to question even if captivity should be employed. Enter the AAEP with the mission to take a fresh look around.

"One stallion jumped out of the corral at the Pine Nuts gather, even though the fence was six feet high. The horse did not appear to suffer any injury when he jumped out, and was allowed to return to the range..." (AAEP report)

According to the AAEP's statistics quoted in the report, as of June 2011, the BLM had 39,948 horses housed in a wide variety of temporary and permanent facilities or with contracted long-term holding ranches.

How the Bureau of Land Management interprets the federal law is a hotly contested issue across the United States. Are government policies "protecting" the wild horses by keeping them in holding pens and farming them out for long-term warehousing? Was it the intent of the law to run these horses on ranches in the Midwest instead of the Western range? Are the gathers in western states that bring in thousands of wild horses each year in the best interest of the horses or of administrative policy? Are the gathers and gatherers guilty of cruel practices that threaten the health and welfare of the horses?

Task force members certainly must have known the issues before they agreed to serve. However, their marching orders clearly stated "It is not the charge of the Task Force to evaluate the BLM program with regard to moral, ethical or economic issues."

Selection of the task force members meant that a wide range of expertise and geographic perspectives were included. None of the veterinarians selected can be described specifically as an academic specialist in equine welfare or behavior or wild horse issues.
"Foot trimming schedules should be created and customized for each facility in accordance with environmental conditions and periodic inspections to reduce the likelihood of excessively long hooves." (AAEP report)

Beginning last fall, the task force visited multiple BLM sites during a six-month period to observe gathers and evaluate conditions at short-term holding and long-term pasture facilities. The task force's data collection was limited to the safety, health status, health management, care, handling and welfare of equids in the BLM program.

"The task force concluded that the care, handling and management practices utilized by the BLM are appropriate for this population of horses and generally support the safety, health status and welfare of the animals," said William Moyer, DVM, AAEP president and a member of the task force, in a press release provided by the AAEP. "However, the task force did see areas that can be improved."

In addition to key recommendations about helicopter herding, anesthesia, biosecurity and other concerns, the veterinarians made suggestions to solidify the footing in the pens at the short-term holding facilities, so that the horses would be able to lie down. 'The overcrowded and wet muddy conditions throughout the pens at Salt Lake Regional Wild Horse and Burro Center in Herriman, Utah, were unacceptable, according to the report. "The conditions posed a health risk for the horses, and did not meet the standard for horse welfare at the time of the task force team visit," the report noted.

An interesting note from the report is that the task force veterinarians thought that the horses in captivity were, in anything, too well fed. "...some horses were fed to excess as evidenced by body condition scores that reflected a substantial amount of body fat," the report explained.

"The BLM staff explained that (hoof) trimming is done on a rotation and that all horses that have long feet are on a waiting list for the trimmer." (AAEP report)

Of particular interest to Hoofcare and Lameness readers is the task force's recommendation that the horses have their hooves trimmed. "Foot trimming schedules should be created and customized for each facility in accordance with environmental conditions and periodic inspections to reduce the likelihood of excessively long hooves," the white paper report informs us.

"A few horses had long feet in need of trimming," the report tells us. "The BLM staff explained that trimming is done on a rotation and that all horses that have long feet are on a waiting list for the trimmer."

All the veterinarians would be able to pick out a horse whose hooves needed to be trimmed. Dr. Moyer in particular is experienced in hoofcare and would have been looking at the hooves with an experienced eye. The fine line between "long" and "too long" would be obvious to Dr. Moyer.

"The observed horses appeared healthy without evidence of chronic injuries, disease or congenital defects other than a small number with a unilateral club foot (about 1% noted at Adobe Town/Salt Wells Complex). Overall hoof condition of the captured horses was judged good."

How do you trim the hooves of a wild horse that has never been touched by human hands, much less had to stand on three legs while its feet are picked up? The report explains:

"The task force teams were told that the average length of stay at the short‐term holding facilities often exceeds 200 days, with a range of 90 to 300 days. Over this confinement period horses do not move enough to wear their feet to a healthy condition as they do on the range.

"Most horses undergo foot trimming during their stay in short‐term holding. The AAEP teams did not observe any hoof trimming but made close inspection of several padded hydraulic squeeze chutes that were used for this purpose.

"When a trim is scheduled, the horse is herded through an alleyway or chute into the squeeze chute. The entry door, sides, floor and exit door of the squeeze chute are hydraulically controlled. Once the horse is enclosed in and restrained with the padded squeeze panels, the chute is rotated 90 degrees onto its side with a separate hydraulic system.

"The foot trimmer accesses the feet through the floor which opens once the horse is in lateral recumbency. Foot trimming is reported to be a quick procedure, accomplished either with hand tools or a special hoof trimming disc on a hand grinder. The squeeze chutes the task force examined were well designed and appeared safe for restraint.

"The foot condition of most horses was good. Less than 5% of the horses at Broken Arrow had long feet in need of trimming. The staff at the sites acknowledged this, saying these individuals were slated for the trimming process soon. The feet were not deemed to be an immediate health risk to the horses."

Dr. Mitchell concluded, "The AAEP will gladly continue if needed as a resource for equine medical expertise to the BLM Wild Horse and Burro program."
